Prop 25: Upgrade to v3

This upgrade fixes an IBC middleware ordering bug that prevented GMP+PFM forwarding for Axelar type=2 (TypeGeneralMessageWithToken) packets. The middleware stack is reordered to ensure forwarding instructions are processed before PFM executes, restoring correct forwarding behavior. This upgrade also establishes the required execution path to support remediation of previously impacted transfers, with any additional operational steps to be communicated separately.
